2778556 Primary Facility 2018-09-20 Routine/Partial Inspection No Violations Noted At the time of inspection, operator had sent in a Notification of Intent to plug this well. Coordinates were verified. GPS could not get better than +/- 20 ft., so operator coordinates are best to use. The road to the well is stoned very well. There were some men onsite to cut some tree limbs down for the rig. Rig was to be moved in later in the day. The well was labeled.
2781087 Primary Facility 2018-09-26 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G had a rig cleaning out the well. Current depth was 2405' and they had tagged bottom at 2385'. TD is reported as 3120'. During the inspection, they had tripped out to run the baler. A lined roll off was being used for waste. The E & S plan was posted at the bridge crossing, which had been approved by the Jefferson County Conservation District. E & S controls looked good.
2788319 Primary Facility 2018-10-11 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite cleaning out well. Several issues have occurred with this well so far. On Sept. 27th, they had lost tools and had to fish out. On Oct. 2nd, they were at 2410' and had some caving issues. They set 2 sxs cement on the 3rd and were at 2406'. By the end of the day on the 9th, they made it to 2411'. They made 6' on the 10th. This morning, liquid gel was added to the hole. Co man believed they could be getting differentially stuck. They are 115' below the packer and about 5' above the top of the Tiona. Plan was to try to clean out the rest of the week and possibly call attainable bottom on Monday.
2792574 Primary Facility 2018-10-17 Routine/Partial Inspection No Violations Noted At the time of inspection, Surefire was onsite running a bond log. So far, 80 sxs cement was placed from attainable bottom (2700') up to 2370'. Log found the bottom of the 4" to be at 2302'. Plan was to try and cut the 4" at 2205' and pull. If that failed, then try at 2015' and pull. Water level was at 46' down.
2797831 Primary Facility 2018-10-25 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite adding pea gravel to the well. It was poured to 3' below ground level, where it will be cut off tomorrow. The monument will be placed above ground. Some extra stone was left onsite if needed. All equipment has been removed from the site.
2798171 Primary Facility 2018-11-05 Routine/Partial Inspection No Violations Noted At the time of inspection, well marker has been set. There was a post above ground with a permit sign next to it. The area has been seeded and mulched. The bridge that was set over Diversified's smaller bridge has been removed.
