2656312 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted At time of inspection, rig awaiting for the arrival of additional equipment prior to the commencement of coring on well 3MHSUT according to operator representative Steve Jager.

Well 3MHSUT is located in a cellar that contains what appears to possibly be water; the Department observed rapid bubbling in this fluid that appears to be originating from the well. The Department immediately forwarded this potential concern to OGI J. Branan for follow-up as appropriate.

2675586 Primary Facility 2017-12-28 Routine/Complete Inspection No Violations Noted This well drilled but not stimulated. Spoke w/CNX Rep. E. Gamble about flange leaking. Gamble is sending a crew for repair.
2682913 Primary Facility 2018-01-10 Routine/Complete Inspection No Violations Noted Inspection is in response to a well cellar leak reported to CNX by the Department on 12/28/2017/8.

Inspection conducted with MRPS Doug Catalano. CNX representatives present were Ryan Coffman, John Hardcastle, Spencer Johnson along with a representative from Cameron Well Heads. Well is being prepped for stimulation activities. All the fluid has been removed from the cellar. Operator reports that the 9 5/8¿ x 5 ½¿ annular pressure is ~2200 psig. 13 3/8¿ x 9 5/8¿ annular pressure is ~400 psig. With the current well head configuration, we are unable to monitor 18 5/8¿ x 13 3/8¿ annulus. Additionally, no one could enter the cellar as it is considered a confined space. With the potential for gas to be present, the cellar atmosphere would need to be evacuated and monitored continually prior to entering. On 12/29/2017, prior to removing the cellar fluids, CNX blew down the 13 3/8¿ x 9 5/8¿ annulus (low volume reported). Gas expression remained present in the cellar after blowing down pressure.
