1963858 Primary Facility 2011-04-06 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Penn view contracted PC Rig #1. Well sign and E&S plan posted on entrance road. Pit was a ligned with plastic. Conductor (20¿) pipe set to 47 ft. First Stringer (13 3/8¿) pipe set at 331 ft. with 300 sks Class A 3% with 6 bbl return. Plug down 1:18 pm. Large amount of coal located in Stratigraphy. Coal showed 390-405 ft. It appears to be in pillar. PC/Penn View started to lose circulation and decided to set ¿mine stringer¿ at 462 ft. Cement should be completed in the afternoon. Anticipated depth is 4550 ft. and expected to perf the shallow zones (Benyord, Speachley, Bradford, and Kane). Cemented job completed by Appalachian. Water encountered at 70¿ and 370ft. Filter fence present on the down gradient side of location. Access road required some work due to weather conditions
1966295 Primary Facility 2011-04-12 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Penn view contracted PC Rig #1. Well sign and E&S plan posted on entrance road. Pit was a ligned with plastic. Conductor (20¿) pipe set to 47 ft. First Stringer (13 3/8¿) pipe set at 331 ft. with 300 sks Class A 3% with 6 bbl return. Water encountered at 70¿ and 370ft. Second stringer (9 5/8¿) set at 462 feet with 200 sks class A no return. Surface Stringer (7¿) set at 1602 feet with 270 sks class A with 7 bbl return. Cement basket set at 88 feet. No reason given for the basket. Under 78.83 (j), cement basket can only be set at location of loss of circulation or mine stringer (set on surface stringer). Current depth was 4570 feet. PC was in the process of setting Production stringer. Wire line was just completed. Filter fence present on the down gradient side of location. Access road required some work due to weather conditions
1966403 Primary Facility 2011-04-13 Routine/Complete Inspection Violation(s) Noted site Id sign and Es plan present.Spud 03/30/11. Drilling completed (rig onsite). Observed accelerated ES issues along access road due to recent heavy rainfalls. Spoke with subcontractor Jason onsite who had plans to divert water along access road into road side ditch towards brush grub line(at reccomendation of dept), ultimately operator may want to install additional rock outlets and other BMP capable of controlling runoff. Temporary BMP¿s were installed on well site according to plan and appeared to be functioning although operator should continue to monitor and maintain all BMP¿s until final restoration activities are completed and implement additional BMP¿s along access road to prevent another possible pollution event. recommended that operator station a broom truck onsite to ensure township roadway is cleaned One lined below grade pit onsite, with adequate freeboard, noted presence of drill cuttings and mud located on well pad surrounding well head. Observed sediment discharge to stream along shared access road.
1975522 Primary Facility 2011-05-23 Compliance Evaluation No Violations Noted CEI inspection for violations on 04/13/11. Since last inspection, site has been re-graded production equipment (except brine tank) installed, drilled pit has been removed contents encapsulated, industrial discharge previously located on well site has been removed & properly disposed of, additional BMP¿s have been added to access road, which operator should continue to routinely monitor/maintain in order to prevent another possible pollution event until final permanent restoration is achieved. Dept recommends that due to steep slope of access road near well pad an additional (permanent) drainage ditch and/or rock outlet(s) be installed to minimize the potential for a subsequent accelerated ES event during heavy or recurrent rainfall events. Reinforced filter fence has been installed near stream (crossing) locations; therefore, previously discharged sediment should be contained by temporary bmps, roadway crowing also completed. Operator should continue to routinely monitor access road. No new viols & oustanding viols corrected at time of inspection
