Mr. Little stated that these issues will be addressed as soon as possible by the site construction crews. At the time of the inspection it was noted that there were no additional erosion & sedimentation issues, apart from what was outlined in the 03/31/10 inspection.
1886776 Primary Facility 2010-05-18 Compliance Evaluation Outstanding Violations - No Viols Req'd Site inspection performed as a follow-up to the previous inspections to evaluate the progress of the operator in addressing the issues outlined in the 03/31/10 inspection and the site meeting on 04/14/10. Found that no actions been taken to address these issues.
1893356 Primary Facility 2010-06-18 Compliance Evaluation No Violations Noted Drilling had been completed, well was not equipped for production. Site had been regraded, seeded & mulched. Noted that there had been additional erosion controls implemented at the culvert outlets to minimize the amount of sediment generated and transported. These additional controls address the issues outlined in the 03/31/10 inspection and 04/14/10 site meeting and inspection. The operator should continue to monitor the site for erosion & sedimentation until site restoration activities are completed and all seeded and mulched areas have achieved 70% uniform vegetative cover.

No equipment present. Well is shut in. No signs of gas leaks at well head. At the time of inspection, no E&S or IW issues. DEP is requesting that Penn View add a well tag to the wellhead.
1941822 Primary Facility 2011-01-10 Site Restoration No Violations Noted At the time of inspection, well drilled, equipped and in production with tagged present. According to records, well completed 03/30/10. No accelerated erosion or sedimentation issues noted. Pits removed and site re-graded. Appears site has been restored as vegetation observed, although area was snow covered and difficult to determine if 70% uniform growth sustained. Brine tank sound. Access road good. A follow inspection will be conducted in spring to ensure that permanent restoration is maintained. Accompanied by WQS Marc Maestra.

Observed large rock pile on well site, recommend operator remove in order to maintain complinace with site restoration. No violations noted at the time of this inspection.
