1920383 Primary Facility 2010-09-17 Routine/Complete Inspection No Violations Noted During inspection, site id with ES plan. Well staked not yet drilled. One unlined below grade pit observed. Operator has plans to encapsulate drill cuttings. Top hole water is to be spread over a undisturbed vegetated area that does not provide direct discharge to any surface waters of the commonwealth, which will be closely monitored. Filter fence and brush/grub line installed below gradient of pad and appears to be properly functioning. Access road is stable, although operator should added additional rock to culvert outlets. It recommended that operator closely monitor all bmps and pit freeboard levels at all times in order to prevent a possible pollution event. Operator should also continue to closely monitor access road entrance to ensure rock and/ or mud does not track on to HWY 295.

Filter fence located topographically down gradient of site pad. Conductor pipe set at 18 ft. (20¿) (sand), 9 5/8¿ pipe set at 378¿ with basket at 325¿ using 75 sks Class A bottom and 110 sks class A basket (balance cement) and back side filled to surface. Mine void at 340¿ to 345¿. 7¿ pipe set at 1184 ft with 110 sks Appellate and 50 sks Class A with 300 # gel and 10 bbl returns. Fresh water noted at 80 ft. Stratigraphy consisted of shale, sandstone, limestone, and coal seams on boring log. Pit covered in 20 ml plastic and no freeboard issues. Permit present. Spud called on 10/11/10 at 7:41 hrs. Spud date was 10/12/10. PC waiting on wireline truck
