Site Id sign and ES plan present. Access road entrance adequately rocked. Access road ok. Well drilled not yet equipped for production. Based on date of completion of drilling well site restoration deadline was 10/26/11. As of this inspection, well site has not yet been regraded, seeded, mulched, or even temporarily stabilized; therefore, operator has failed to restore well site within nine months of completion of drilling. Minor accelerated erosion and sedimentation concerns noted below well pad. Erosion and sedimentation concerns observed onsite Dept observed that various sections of compost sox are in need of maintenance, due to buildup of sediment. Operator agreed to repair and implement additional bmp's( filter fence and compost sox) in order to prevent a possible pollution event until final permanent restoration is achieved.

Site has been seeded/mulched with approximately 70% vegetative cover, department recommends operator reseed/mulch in early spring in order to maintain uniform vegetative cover capable of (permanently) resisting erosion, especially in cut slope location where former drill pit located. No accelerated erosion or sedimentation issues noted at time of inspection. Operator shall continue to closely monitor and maintain well site and access road until permanent uniform vegetative cover is established at which time all temporary bmp's shall be removed. No violations noted.
