Site was properly signed with E&S plan available for review. Access road entrance was adequately rocked. Noted some disturbed areas along access road across form church above well pad that it is recommended be reseeded promptly. Filter fence was installed down gradient from well site and appears to be functioning properly. No accelerated erosion or sedimentation issues were noted on well site. Pit appeared structurally sound and sustained two ft freeboard. Noted a drainage pipe on right down gradient side of pad near church parking lot, appears to be possibly a field drain that is not noted on E&S plan. Further investigation will be done to accurately identify the pipe and its function. Also noted that approval was given for the well to be drilled at a location closer than 200ft from and existing building/water supply with the exception written owner consent is given to do so.

Brine tank appears to be sound. Tag is present. Site is covered by a heavy layer of snow making assessment of E&S controls difficult. Site will be re-evaluated when weather conditions improve.
