Site properly signed with E&S plan onsite. One drill pit onsite with adequate freeboard. Pad was rocked. Filter fabric fence properly installed and functioning. No accelerated erosion or sedimentation issues observed with site or access road.
1915663 Primary Facility 2010-09-22 Site Restoration No Violations Noted At the time of this inspection, well drilled, properly tagged and in production. According to records, drilling to a total depth of 4093ft was completed on 12/14/09. Well site restored to approximate pre existing conditions (rock base material), little vegetation was utilized due to the abundance of rock in base material prior to construction. No accelerated erosion and sedimentation issues were noted. Brine tank appears sound. No IW issues observed. Recommend that operator seed and mulch cut slope behind well pad in order to ensure permanent stability over a long period of time. Accompanied by WSQ Dana Burk during this inspection.

Access road is in good condition. Brine tank appears to be sound. Site is on gravel, limited vegetation. Dept. did not observe any E&S issues.
