Department contacted operator rep Stan Berdell by phone from site to inquire about dripping brine tank piping. Mr. Berdell stated well tender will be contacted immediately and follow up with Department. 10:40: Mr. Berdell contacted Department by phone. Mr. Berdell stated that well tender will fix brine tank pipe today. Department recommends operator follow up with Department regarding type of fluid released and amount of fluid released. 10:54: Operator rep Paul Kimmell contacted department by phone. Mr. Kimmell stated well tender will be on site within 30 minutes and will follow up with Department. Violation being issued for operator release of production fluids. A follow up inspection will be conducted at a later date.

2677971 Primary Facility 2018-01-04 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2677770 where a violation was issued for operator failure to collect production fluids.

Operator rep Paul Kimmell contacted department by phone at 17:49 on 1-3-18 and left message. Message stated well tender Randy McCoy was on site and unable to determine where release was occurring. Also, that the line used to empty brine tank had a dug out area under it that may have been caused by deer. Some fluid in this area may have dripped at one point and attracted the deer. No dripping was seen by Mr. McCoy. Mr. Kimmell also stated in his message that well tender records indicate no loss of fluid from the tanks. Mr. Kimmell offered to schedule a field visit with well tender and Department to ensure operator is addressing Department's concerns. Previously issued violation for operator release of production fluids remains outstanding. A follow up inspection will be conducted at a later date

Department returned operator phone call from 1-3-18 and spoke with Paul Kimmell. Mr. Kimmell stated that there may have been a small release of fluid when the brine tanks were being emptied. The tanks have not been emptied for some time so the date of release is unknown. Mr. Kimmell stated the soil will be excavated and the area remediated. He will follow up with Department. Previously issued violation for operator release of production fluids remains outstanding. A follow up inspection will be conducted at a later date.

Department received operator response to NOV (photo #1). Previously issued violation still exists for this site. A follow up inspection will be conducted at a later date.

At time of inspection, impacted soils appear to have been excavated and earthen dike rebuilt. Based on operator response and submitted photos (inspection record #2682425), and this field inspection, operator appears to be in compliance at this time. Violation issued on 1-3-18 has been corrected. Follow up inspections will be conducted to ensure site is seeded and mulched and that site reaches permanent stabilization. Violation(s) may be forthcoming based on follow up inspections.

2826622 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted On 1/11/19 at 1424 hrs., I conducted a routine inspection of BLX, Inc.'s R Sleppy 1-86.6well (API 063-36534) in Cherryhill Twp., Indiana Co. Snow cover prevented proper evaluation of site stabilization. The well pad appears to be restored and permanently stabilized. The well is properly labeled and equipped for production. A site ID sign is posted and a well tag is present at the well head. The production fluid tank is located within secondary containment. There are no violations noted for this inspection.
