Well is equipped for production. Tank appears sound. Vegetated dirt dike around tank. Access road entrance is adequately rocked and stable. Work pad on site is stabilized with rock and vegetation. Entire remainder of site was plowed and planted by surface owner. Additional rock has been installed on access road, ditch lines, and culvert inlet and outlets. Silt fence has been removed and disturbed areas along access road seeded and mulched. These areas are fairly well vegetated. Access road is stabilized. Work pad and portions of site not disturbed by farming operations are stabilized. Site is in compliance. No violations found. Planted fields adjacent to site show multiple signs of erosion. E&S issues in this area appear to originate from the fields.

2089197 Primary Facility 2012-08-01 Complaint Inspection No Violations Noted At the time of this inspection access road appears adequately rocked, there appears to be evidence of a prior erosion concern with mud on township roadway. Well drilled in production. Brine tank appears sound. No accelerated erosion or sedimentation issues on well site. Recommend that operator place "Barrels" on containment in order to prevent a possible pollution event. Site restored. adequate vegetative growth on previously disturbed areas and pad adequately rocked. Recommend operator examine culvert ditch(s) and address issue of accelerated erosion and sedimentation concerns by catching and filter runoff in to culvert outlets. Department has plans to contact operator and discus possible measures to address increased runoff during heavy rain events; it appears some run-off maybe also be coming from surrounding farm field. Similiar complaint recvd July 2010 & looked at by Dep. Insp result pending operator response. 10/2/12 operator successfully implemented additonal bmp's and minimized ES. Onsite meeting to discuss efforts to address storm water run-off &sedimentation entering township road from well site access road. operator agreed to implement additional storm water management practices in order to minimize the potential for accelerated erosion and sedimentation issues. Tentative plan addition of: 24' compost sox placed in field along access road in order to prevent sedimentation from field along/onto access road,iversion ditch and sediment trap will be install near top of access road at turn,Water bars and additional culverts will be added along access road,relocate culvert and install sediment basin or check dam at outlet in order to prevent such a discharge, and Roadside ditch will be clean out, deepened and re-filled with rock. Based on todays inspection and in light of recent rainfall events, it appears that operator has succesfully implemented permanent bmp's capable of resisting erosion and accelerated sedimentation. Therefore, no violations noted at thte time of this inspection.Department reccomends operator continue to mointor and maintain bmp's partilcuarlly after heavy rain events.

Operator representative Spencer Johnson contacted the Department via phone. In order to prevent any potential pollution events from occurring, maintenance of the access road is scheduled to begin Friday 8/18.

As a result of the concerns expressed in inspection record 2627040, the operator will continue to routinely monitor and maintain the access road as necessary. The operator anticipates plugging of the well. Upon plugging, the access will be restored unless the landowner wishes for it to remain.
