1817416 Primary Facility 2009-07-31 Drilling/Alteration No Violations Noted Currently, well completed to a depth of 4,000 ft. below grade. Well site was identified with well sign. Heavy rains casusing visible E&S concerns at the time of inspection with sediment occurring on the road and flowing into streams. Requested Cambria DEP water quality personnel to perform an evaluation. No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Drilling was completed and rig moved to Little #5. Well completed with 7¿ pipe converted to 4 reducer to shut off valve. Fracturing of the well is scheduled for August 4th. Met with Rick Blazek and discussed access road and runoff.
1817936 Primary Facility 2009-08-03 Routine/Complete Inspection Violation(s) Noted Well is in post drilling phase of construction. Erosion and sedimentation noted on the access road at the time of inspection. The E&S controls were found to be inadequate on the access road. Pit liner embedded properly and pit is maintaining 2 foot freeboard requirement. It is suggested that the operator install broad based dips with rock outlets on the access road to minimize accelerated erosion and sedimentation. In addition the all culvert inlets and outlets should be stabilized with rip rap to reduce sedimentation. A 50 ft min entrance should have #1 ASHTO rock as stated in the E&S plan that is located on site. The operator should continue to monitor the access road and well site for erosion and sedimentation until final restoration activities are complete.
1827093 Primary Facility 2009-09-03 Compliance Evaluation Outstanding Violations - No Viols Req'd Well is in production. The site has been regraded, seeded, and mulched. The broad-based dips have been installed on the upper portion of the access road leading to Little #1, however the lower portion of the access road near the stream should be addressed to minimize erosion and sedimentation before the violation can be closed. The installation of E&S controls were discussed with Brad Little during an on-site meeting. There are no new violations at the time of inspection. 1965616 Primary Facility 2011-04-13 Site Restoration No Violations Noted At the time of inspection, well drilled and in production. Well tag present. According to records, well completed on 07/23/09 to a depth of 5050ft. Brine tank appears sound, no IW issues noted. Site adequately and uniformly restored with >70% uniform perennial vegetative cover. No accelerated erosion or sedimentation issues observed with site or access road to this well site.
