Well tag present and legible. Department observed fluid dripping from brine tank valve. Small patch (approx 15 inches X 15 inches) of stained gravel located under the valve. Department contacted Operator by phone from site to report findings and recommend well tender visit site. Operator stated well tender will be sent to site immediately. 12:00. Well tender Randy McCoy met Department at site. Mr. McCoy stated the valve appears to be broken. He also stated tank appears to be one-third full and was last emptied sometime last year. He visits this site once a week and was last on site 'a few days ago' and did not notice any leaking fluid at that time. Mr. McCoy stated he would excavate visibly impacted soils immediately and place a bucket under valve to catch dripping fluids until tank can be emptied and valve replaced. 2565045 Primary Facility 2017-02-17 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection to inspection record # 2564345 where violation issued for discharge of brine or other fluids on or into the ground or into waters of the Commonwealth.

Operator Rep Randy McCoy contacted Department by phone and left message. Mr. McCoy stated that the brine tank has been emptied and and the broken valve has been replaced. Mr. McCoy also stated that the amount of impacted soil was approximately half a 5 gal bucket. Department recommends Operator dispose of impacted soil by a method permitted by law. Previously issued violation still exists for this site. A follow up inspection will be conducted at a later date.

2570205 Primary Facility 2017-03-09 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection to inspection record # 2564345 where violation was issued for discharge of brine or other fluids on or into the ground or into waters of the Commonwealth.

Department received response to NOV from Operator. Operator states that freezing water damaged valve on brine tank. Operator also states that the impacted soil was removed on 2-15-17 and that the faulty valve was replaced on 2-16-17. Department recommends Operator dispose of impacted soil in a manner permitted by law.

A follow up inspection will be conducted at a later date. Previously issued violation still exists for this site.

2571456 Primary Facility 2017-03-13 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to inspection record # 2564345 where a violation was issued for discharge of brine or other fluids on or into the ground or into waters of the Commonwealth.

At time of inspection it appears a new valve has been placed on the brine tank. The brine tank/valve appear structurally sound. Excavated area below valve has been rocked and appears pemanently stabilized.

Previously issued violation (25 Pa. Code § 78.57(a)) for this site has been corrected.
