Operator rep Sam Kimmel contacted Department by phone. Mr. Kimmel stated he was aware that he was late in responding to the NOV and that his response will be emailed to the WQS in the near future. Mr. Kimmel stated that no E and S plan was developed because no earth disturbance was conducted. No pit was dug; all fluids/materials normally placed in a pit during plugging were contained in tanks manifolded together. After plugging was completed, all equipment was removed and the site was seeded and mulched. Mr. Kimmel also stated that he was issued a GP for the stream crossing and will email a copy to the WQS. Department recommends operator ensure GP is on site until permanent stabilization has been achieved. A follow up inspection will be conducted at a later date. |
