SWRO notified of spill at 1510 by Opr rep Tom Livingston.WQS contacted Mr. Livingston via phone at 1535.Mr. Livingston stated that, at 1345, a skid-steer hit prod tank, puncturing it, causing the release to occur.Due to present prep activities for well stimulation, vac truck on-site at time of release.Fluids immediately suctioned up and remaining soil/slush scraped up and placed into lined completions pit.Soils placed on liner awaiting placement into roll-off box.Opr utilizing conductivity probe as guidance to ensure all contaminated soils removed.Approx an inch of prod water lost from tank.Tank no longer leaking.Mr. Livingston reports all visibly impacted soils have been removed.No waterways affected.Due to immediate clean-up efforts, Mr. Livingston stated that prod fluids were on ground for min amount of time.It appears Opr is handling situation according to policy and a FUI will be conducted in future to assess any environmental impacts and issue any viols

Mr. Livingston states remedial actions to be conducted by removing all visually impacted soils and utilizing a conductivity meter as guidance. Department recommended that remedial efforts include either the utilization of Act 2 or sampling under the Alternative Remediation option, as outlined in the current spill policy.Violations are being issued for this reportable release of residual fluids under 25 PA CODE 78.54, 78.57A, 91.34.
