2592879 Primary Facility 2017-05-03 Routine/Complete Inspection In Compliance with Policy Admin inspection conducted in response to inspection record 2589621. At 0759, operator representative Vern Shumaker contacted the Department via phone.

Per Mr. Shumaker: He is en route to the well site. Booms and absorbent pads will be utilized to control and remediate the release and all impacted materials will be excavated. The tank will be emptied and removed. A quantity released will be provided to the Department.

Mr. Shumaker stated that he believed the tank to be empty, so it must be residual oil within the tank that is leaking.

2598015 Primary Facility 2017-05-15 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to inspection record 2589621.

At 1032, operator representative Vern Shumaker notified the Department via voicemail that a water truck is going to empty the remaining fluids from the tank and then the tank will be removed from the well site. Operator appears to be in compliance with policy at this time. Inspection record 2589621 remains pending upon operator response and further investigation.

2602583 Primary Facility 2017-05-26 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to inspection record 2589621.

At 0719, Department spoke to Operator representative Vern Shumaker via phone.

Per Mr. Shumaker: The tank has been emptied and is scheduled to be removed from the location early next week. Mostly rainwater was released because it had been entering through the top of the tank. An estimated release of 20 gallons oil and water occurred. At this time all visibly impacted soils have been excavated in accordance with the Department's Spill Policy. Additional excavation may be necessary upon removal of the tank.

According to Mr. Shumaker: The well site is checked twice a month and had not been leaking during the last site visit. The source of the leak was due to a leaking 3/8 fitting on the sight glass. The leak was initially stopped by replacing the fitting with a plug until the tank could be emptied and removed.

Mr. Shumaker agreed to provide pictures to the Department.

Operator representative Vern Shumaker notified Department that the tank has been extracted from the well site, the contaminated soils excavated, and there is no indication of saturation of oil.
