2553301 Primary Facility 2017-01-09 Administrative/File Review In Compliance with Policy At 1531, Operator representative Erik Greenert notified the Department via phone that water is being discharged from between the 10 3/4" and 13" casings of a well (063-24161). Per Mr. Greenert, the Operator was killing the well for plugging operations and believes that while doing so, an artesian was exposed. The well is located in a previously mined out area. Field readings indicate the pH to be 7.0 and the conductivity to be 800µmhos/cm. The water is clear.

Mr. Greenert reported that a stream is located approximately 50-60 yards from the well and stated that the discharging water is flowing to the opposite side of the site from the well. The water is contained on-site at this time and is estimated to be discharging at a rate of less than 1 gpm. The water has traveled approximately 50 feet from the well although it is difficult to verify due to the current on-site conditions (i.e. snow, snowmelt, precipitation). 2553312 Primary Facility 2017-01-10 Administrative/File Review In Compliance with Policy At 0828, Operator representative Erik Greenert contacted the Department via phone as a follow-up to inspection ID 2553301 for the reported discharge of water from between the casings of a conventional well.

According to Mr. Greenert: The discharge is still occurring at a rate of less than 1gpm. Field readings show conductivity levels of approximately 600 µmhos/cm and a pH of 7.0. The water is clear. There is a trench currently collecting the water and a vac truck has been pumping the water from the trench as it fills. Water is seeping out from the edge of the pad, however it is difficult to discern whether the water is from the discharging fluids from the well or from the current snowmelt. Per Mr. Greenert: The seeping water appears as if it may be heading in the direction of the nearby tributary to Yellow Creek. Again, it is difficult to discern whether the water is from the discharging fluids from the well or from the current snowmelt. The seep is approximately 30 yards from the tributary. The seeping “wa

2553362 Primary Facility 2017-01-10 Administrative/File Review In Compliance with Policy At 1308, Mr. Greenert contacted the Department via phone as a follow-up to inspection ID 2553301 for the reported discharge of water from between the casings of a conventional well.

Per Mr. Greenert: The discharge has ceased as a result of capping off the well. A pipe was previously placed between the annuluses where the water was discharging so that it could be directly removed by the vacuum truck without discharging onto the ground. There is no longer a seep. According to Mr. Greenert: The discharging water did not reach the tributary. No erosion concerns exist as a result of this discharge. Mr. Greenert stated that water was being pumped into the production string when water began discharging from between the casings. Mr. Greenert believes the Operator is going to continue plugging operations. The Department recommended that the location of the seep is monitored should seeping begin again. Mr. Greenert agreed to keep monitoring this area, but does not believe that the seep was from the water discharge

2553365 Primary Facility 2017-01-10 Administrative/File Review In Compliance with Policy At 1507, the Department contacted Operator representative Erik Greenert via phone as a follow-up to inspection ID 2553301 for the reported discharge of water from between the casings of a conventional well.

Mr. Greenert stated that there continues to be no discharge from the well and no seep. The third party lab is en route to collect samples. Pictures are to be provided to the Department later this afternoon or tomorrow morning. The Operator appears to be in compliance with current policy at this time. A follow-up inspection will be conducted to assess any environmental impact and/or issue any associated violations as necessary.

2553400 Primary Facility 2017-01-11 Incident- Response to Accident or Event No Violations Noted On-site inspection conducted in response to admin inspection 2553301 for the reported discharge of water from between the casings of a conventional well.

At the time of inspection, rig on-site. Operator representative Rich Leightner on-site.

Per Mr. Leightner: Plugs are currently being set. The well was capped off to prevent the discharge of water from continuing. The water was discharging from between the 8 5/8” and 13 3/8” casings. The Operator believes the discharging water was coming from between 30-60 feet down. Field readings collected by Mr. Leightner showed the discharging water to have a pH of 7.0 and a conductivity of 813 µmhos/cm.

During this inspection, the Department did not observe the water discharging from the well. The Department did not observe any adverse environmental impacts at this time that may have resulted from this reported discharge.

2553407 Primary Facility 2017-01-12 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to admin inspection 2553301 for the reported discharge of water from between the casings of a conventional well.

At 1053, the Department called Mr. Greenert and left a message. At 1317, Mr. Greenert returned the Department’s phone call. Per Mr. Greenert, the discharge remains stopped and no more water has seeped from the edge of the well pad. Based on Mr. Greenert’s observations while the seep was active, he does not believe that the seep was related to the discharge. Mr. Greenert does not know if the casing is compromised, but believes a cement bond log is being run. Samples were collected by a third party lab. The Department recommended to Mr. Greenert that the appropriate actions are taken as necessary in order to prevent any disturbance and/or discharge to Waters of the Commonwealth.

2554315 Primary Facility 2017-01-20 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to admin inspection 2553301 for the reported discharge of water from between the casings of a conventional well.

At 1141, the Department spoke to Operator representative Erik Greenert via phone.

Per Mr. Greenert, the discharge has remained stopped since it was originally ceased by welding a plate onto the casing. Plugging of the well has been completed. There was no discharge of fluids during the plugging process.

Mr. Greenert stated that the sample results have been received from the third party sample collection of the discharging fluids. According to Mr. Greenert, the discharge appears to have been freshwater from an artesian. The Department requested that the sample results are submitted to the Department. Mr. Greenert agreed to provide these sample results to the Department.

2567992 Primary Facility 2017-02-21 Administrative/File Review No Violations Noted Admin inspection conducted in response to admin inspection 2553301 for the reported discharge of water from between the casings of a conventional well.

Sample results submitted to Department by Operator representative Erik Greenert. Based upon review of analytical results, sample profile does not appear to be indicative of pollutional fluids. No violations noted as this time.

2571831 Primary Facility 2017-03-02 Routine/Complete Inspection No Violations Noted At the time of inspection, plugging appears to have been completed. One frac tank on-site.

Temporary BMPs appear to be installed in accordance with the on-site E&S plan. Topsoil pile temporarily stabilized with mulch. Pad graveled, currently stabilized. Access road stabilized. No accelerated erosion and/or sedimentation issues observed during this inspection. The Department’s regulations provide that a well site must be restored within 9 months of completion of plugging of a well. The Operator is within this deadline at this time.

2639738 Primary Facility 2017-09-14 Routine/Complete Inspection In Compliance with Policy At time of inspection, well reported to be plugged. It appears that PennDOT is performing earth disturbance activities that is encompassing the former location of the well site.

Following this inspection, the Department contacted operator representative Gregory Rothschling via phone and left a message to follow-up on this observation.

2640099 Primary Facility 2017-09-19 Administrative/File Review In Compliance with Policy Admin inspection.

Operator representative Morgan O'Brien provided the Department with documentation demonstrating that this gas well was plugged per an agreement with PennDOT as PennDOT has a planned project for this location. It appears that the earth disturbance activities for this planned project were underway during the Department's previous inspection on 9/14/2017 (inspection record 2639738).

2685680 Primary Facility 2018-01-23 Site Restoration No Violations Noted Inspection conducted in response to a Well Site Restoration Report received by Department on 6-19-17. At time of inspection well is plugged; it appears a buried plate is installed. According to inspection record #2640099 this well was plugged to allow PennDOT to complete a highway project. At this time PennDOT is actively working at the site.

This Restoration Report is being approved without the required vegetative growth due to the site being encompassed in the PennDOT highway project area.
