2745058 Primary Facility 2018-06-21 Incident- Response to Accident or Event No Violations Noted The well is a conventional stripper well producing gas and small amounts of brine with little to no oil. It is located on Game Commision property and is difficult to access. The GPS of the well location is accurate but it took a while to find a way in to the well. You have to go to a Game Commision gate (normally closed to the public and locked) and follow the road approx. 0.9 - 1.0 miles back to the well (marked with orange tape on both side of the road and on the right as driving in). The access gate is located at N 40.83150 W 78.87024. It should be open until Mr. Shumaker is done with his repairs and/or the crew working in the woods is finished.

The 4 1/2" well head, which was attached to a production line was sheared off of the casing when a construction crew drove over it with a blade being used to clear small trees and brush. The crew was also spraying an herbicide designed to kill the undergrowth. The herbicide appears to be an emulsion of water, diesel and herbicide. If you look closely in the pictu

2746276 Primary Facility 2018-06-22 Incident- Response to Accident or Event Violation(s) Noted Inspection conducted in response to a notification that a wellhead was hit and broke off by a logging company.

At the time of this inspection, the well has not yet been repaired. The well is bubbling. The bubbling is audible and gas fumes can be seen exiting the well. There is a strong gas odor observed. At this time, no fluids appear to be discharging from the well.

There is no one on-site during this inspection and it is lightly raining.

2748870 Primary Facility 2018-06-25 Follow-up Inspection Outstanding Violations - Viols Req'd FUI to inspection record #2746276 where violations were issued for failure to control and dispose of fluids and failure to take necessary corrective actions during a release.

At time of inspection well appears to be shut in (photo #1). No active release observed at this time. Some stressed vegetation present near well head area (photo #2). Department contacted operator rep Vern Shumaker by phone. Mr. Shumaker states a logging crew struck the well head. Mr. Shumaker also states the crew was spraying herbicides to kill the undergrowth. Mr. Shumaker states the well head was repaired on 6-22-18 and that a backhoe will be on site possibly tomorrow (6-26-18) to excavate impacted soils. Department recommends operator follow up with Department regarding remediation activity at site. Violations issued on 6-22-18 remain outstanding. A follow up inspection will be conducted at a later date.

2753783 Primary Facility 2018-07-18 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2746276 where violations were issued for failure to comply with 78.66(e), failure to comply with 91.34 (a), and failure to comply with 78.54.

Department received operator response to NOV (photo #1). Violations issued on 6-22-18 remain outstanding. A follow up inspection will be conducted at a later date.

2758434 Primary Facility 2018-07-31 Compliance Evaluation No Violations Noted CEI inspection to inspection record #2746276 where violations were issued for failure to comply with 78.66(e), 91.34(a), and 78.54.

At time of inspection well appears equipped for production. Area of release observed in the above inspection record appears seeded and mulched. Vegetation beginning to grow. Department recommends operator continue to monitor site to ensure vegetation reaches >70% uniform growth. Violations issued on 6-22-18 have been corrected.
