2754254 Primary Facility 2018-07-20 Drilling/Alteration No Violations Noted The well was spud on 7/17/18. The operator has set a 24" conductor pipe at 80'. The conductor pipe has been driven/drilled to depth.
2827719 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted On 01/09/19 at approximately 0835 hours I conducted a routine complete inspection of the site in response to a notification from Greylock that a Modular Above Ground Storage Structure was being constructed. As of the time of inspection the MASS had been constructed.

An area near the MASS showed signs of elevated conductivity compared to background (1.07mS vs 0.2mS). It is recommended the area be assessed for potential produced fluid impacts.

A water line between the MASS and well pad appeared to have been damaged by a vehicle (see photographs). It is recommended the line be pressure tested or repaired prior to use.
