2809949 Primary Facility 2018-11-26 Incident- Response to Accident or Event In Compliance with Policy On 11/22/18 at approximately 0952 David Guidas of EQT reported an impaired water spill on the Shipman well pad.

On 11/26/18 at approximately 1200 hours I conducted an incident response inspection. I spoke via phone with David Guidas who indicated 10 gallons of produced fluid was spilled when a line containing impaired water was removed from a frac tank. Due to teardown activities at the site the secondary containment had a hole that allowed the fluid to reach the ground surface. The spill was remediated by SPSI. The spill has been addressed in compliance with DEP spill policy.

2821436 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted Well currently being flowed back or prepping for flow back after drill out operations. On site inspection found no apparent violations. NOVIO
