2709505 Primary Facility 2018-03-15 Preoperation Inspection No Violations Noted On March 15, 2018, the Department performed an inspection of the EQT/Rice Energy Beta Cameron well site. Present at time of inspection was Rex Nelson (EQT). The site is located in Richhill Twp., Greene Co. The inspection was related to a request for a pre-operational inspection of the site before the operator commences drill operations.

The operator was stoning the well pad at time of inspection in preparation to begin drilling operations on the site. The operator was in process of stabilizing the site with seed and mulch. Construction of the PCSM structures has begun and the area is being temporarily stabilized.

6 wells have been permitted for the but none have been drilled at time of inspection. The inspection was a pre-spud inspection for the 6 permitted wells.

2710914 Primary Facility 2018-03-27 Drilling/Alteration No Violations Noted The well was spud on 3/16/18. Operator drilled a 30-inch borehole to 40-feet. A 26-inch conductor pipe has been set at 40-feet and cemented through tubing to the surface. A well cellar has been installed around the conductor pipe.

The operator drilled a 20-inch borehole adjacent to the well cellar and set and cemented a 16-inch mouse hole pipe to the surface.

2717513 Primary Facility 2018-04-12 Drilling/Alteration No Violations Noted The well was spud on 3/16/18. Operator drilled a 30-inch borehole to 40-feet. A 26-inch conductor pipe has been set at 40-feet and cemented through tubing to the surface. A well cellar has been installed around the conductor pipe.

2734913 Primary Facility 2018-05-29 Drilling/Alteration No Violations Noted Patterson rig 349 on site. Operator resuming drilling on well 059-27436.
2803859 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted On October 22, 2018, the Department performed an inspection of the EQT/Rice Energy Beta Cameron well site. Present at time of inspection was Jamie Davis (EQT). The site is located in Richhill Twp., Greene Co. The inspection was related to a request by the operator to meet on site and review some proposed minor modifications to the permit for repairs to the PCSMs on site.

The operator has requested for some minor modifications to the ESCGP2 permit to place rock aprons in the cut slopes of a wet pond and rain garden which failed (PCSMs). The operator has also requested to extend rock aprons from culvert discharges down the fill slope to the PCSMs. The minor modifications were reviewed and approved by the Department in the field.

3 wells (8,10, and 12) have been completed and are equipped for production on the well pad.

3 wells (2,4, and 6) have been drilled to TD, but have not been hydraulically fractured.
