2572594 Primary Facility 2017-03-17 Preoperation Inspection No Violations Noted On Tuesday, March 14, 2017, I conducted a pre-operation inspection of the Huffman Well Pad located in Center Township, Greene County at the request of the operator in accordance with 58 Pa. C.S. Section 3258 (a.1). Mr. John Sandy, representing EQT Production Company, was also present during the inspection.

The Act 9 site ID sign was clearly legible at the site entrance and all required documentation was available on-site. The access road and the well pad were in good condition.

All perimeter BMPs were in place, functional and installed according to plans. A few areas were supplemented with additional filter sock which were red-lined onto the plans to document the additional controls. The well pad and access road slopes exhibited various stages of vegetation and stabilization has been achieved through a combination of vegetation, seed and erosion control matting. The operator should continue their periodic monitoring program and immediately address areas exhibiting signs of accelerated erosion and sedi
