2635691 Primary Facility 2017-09-08 Routine/Complete Inspection No Violations Noted On Friday, September 8, 2017, I conducted a routine-complete inspection at the Rough Hollow Well Site located in Washington Township, Greene County.

The Act 9 sign and a sign displaying gas well API numbers were in place and clearly legible at the site entrance. All required documentation was available on-site.

The access road and the well pad were in good condition and overall, the site is well vegetated with uniform perennial vegetation except for a few spotty areas of growth on the soil stockpile behind the AST pad.

At the time of inspection, Cellar Tech was on-site advancing mouse holes next to each conductor in preparation for moving a triple rig on-site within the next week or so. Other contractors were on-site building the GPUs and preparing the production tanks and the associated secondary containment.

No violations were identified during the inspection.
