On 09/28/16 at approximately 0800 hours I conducted a pre-op inspection of the EQT Rivers Unit. The Act 9 sign was in place. All required documents were present on the site.

The well pad had been completed and all BMPs installed. EQT was currently installing the freshwater impoundment and AST pad at the time of inspection. There were no signs of erosion and all the BMPs appeared to be in good condition.

Two red line changes were approved at the time of inspection. The changes were to the location of two pad sumps on the pad. The movement of the sumps does not change the drainage areas. There were no violations observed at the time of inspection.

Due to restricted view of wellheads it is impossible to determine production capability of any well. Inspection of location showed no apparent violation.
2846649 Primary Facility 2019-02-21 Compliance Evaluation No Violations Noted On 12/29/19 EQT observed a reported 10-15 gallon brine spill of unknown source at the site. An NOV was issued and remediation was conducted.

On 02/21/19 at approximately 1100 hours I conducted a compliance evaluation inspection. The impacted sump had been excavated and replaced. The rip rap at the sump pipe outlet appeared to have been replaced, as it did not exhibit staining observed in the previous inspection. It appeared the spill had been remediated. There were no violations observed at the time of inspection.
