On 09/28/16 at approximately 0800 hours I conducted a pre-op inspection of the EQT Rivers Unit. The Act 9 sign was in place. All required documents were present on the site.

The well pad had been completed and all BMPs installed. EQT was currently installing the freshwater impoundment and AST pad at the time of inspection. There were no signs of erosion and all the BMPs appeared to be in good condition.

Two red line changes were approved at the time of inspection. The changes were to the location of two pad sumps on the pad. The movement of the sumps does not change the drainage areas. There were no violations observed at the time of inspection.

2771762 Primary Facility 2018-08-29 Incident- Response to Accident or Event Violation(s) Noted On 08/28/18 at approximately 0530 hours EQT reported a spill of 100 gallons of produced water on the well pad.

On 08/29/18 at approximately 1145 hours I conducted an incident response inspection. EQT staff were present, but for an unrelated project and they did not have knowledge of the spill details. I contacted Corey Giles, Senior Environmental Coordinator for EQT, who indicated the release was from GPU unit #2. The spill area was to the rear of the GPU and Giles indicated a sump was dug, since elevated conductivity fluid continued to be observed. The fluid was being pumped from the sump to a frac tank on site. Violations of SWMA 301 and 78a57a occurred as a result of the spill.

The GPUs on site did not have secondary containment and the earliest SPUD date for wells on the pad was 10/26/2016. This is a violation of 78a64a(b), as the GPUs were installed after October 8th, 2016.

The frac tank the spilled production fluid was being pumped too was also not managed within secondary containment. This is an

2790767 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted One of a series of wells with all wellhead equipment under ground level in basement and covered by grate. No wells onsite are identified by API Number or well name. All but two cellars are filled with water restricting OGI ability to inspect. No bubbling noted on any flooded basement. 5 of 15 wells are listed as inactive, remainder listed as active. Due to restricted view of wellheads it is impossible to determine production capability of any well. Inspection of location showed no apparent violation.
2846631 Primary Facility 2019-02-21 Compliance Evaluation No Violations Noted On 12/29/19 EQT observed a reported 10-15 gallon brine spill of unknown source at the site. An NOV was issued and remediation was conducted.

On 02/21/19 at approximately 1100 hours I conducted a compliance evaluation inspection. The impacted sump had been excavated and replaced. The rip rap at the sump pipe outlet appeared to have been replaced, as it did not exhibit staining observed in the previous inspection. It appeared the spill had been remediated. There were no violations observed at the time of inspection.
