2574712 Primary Facility 2017-03-13 Routine/Complete Inspection No Violations Noted On March 13, 2017, the Department performed an inspection of the Rice Energy Polar Vortex well pad site. The site is located in Jackson Township, Greene county. The inspection was a routine complete inspection during drilling activity.

The operator is currently completing installation of secondary containment and is moving rig on site to complete drilling of lateral sections on the four wells on site.

Well drilled to TVD and awaiting drilling resumption to complete drilling to TD.

2708380 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted On March 8, 2018, the Department performed an inspection of the EQT/Rice Energy Polar Vortex well pad site. The site is located in Jackson Twp., Greene Co. The inspection was CEI related to violations noted on 9/28/17 and 12/7/17.

The operator has finished flow back operations on the recently hydraulically fractured wells on the site. The flow back equipment has been removed. The operator has remediated the two reported frac fluid releases on the well pad. The operator has recently completed construction of the PCSM structures on the site.

4 wells have been drilled to TD on the site, and have been equipped for production.
