The well site was found to be in violation of 25 Pa. Code § 102.22(a) relative to site stabilization due to the development of a potential landslide along the access road. The fracture where movement has started extends up the access road across the side berm and down slope. Corrective action is warranted.

No violations noted during the inspection.
2557915 Primary Facility 2016-12-22 Routine/Complete Inspection No Violations Noted On 03/30/16 an inspection was conducted by Water Quality Specialist John Parry of the Coastal Greene well site. During this inspection, Perry noted that a section of the access road was separating and forming a slip. An NOV was issued for 102.22(a) pertaining to site stabilization, where stabilization is defined as the proper placing, grading, constructing, reinforcing, lining, and covering of soil, rock, or earth to ensure their resistant to erosion, sliding, or other movement.

On 12/22/16 at approximately 1130 hours I, along with Compliance Specialist Kevin Maskol, met with representatives of ECA to perform an inspection of the Coastal Greene well site. The area where the access road and the fill slope to the north of the access road settled, cracked, and slipped was observed. At the time of the inspection ECA had nearly completed the repair. According to John Sollen of ECA, the area was excavated to a depth of 40’, a rock curtain drain was installed, and the fill was keyed in and placed back in lifts.

2689025 Primary Facility 2018-01-19 Routine/Complete Inspection No Violations Noted The Act 9 was in place and all required documents were present at the site. The access road had previously failed, but had been repaired and appeared to be in good condition. The site was snow covered and site vegetation could not be inspected. The brine tanks had been partially installed. The wells had been spud. There were no violations observed at the time of inspection.

2733895 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted On May 1, 2018, the Department performed an inspection of the Greylock Production LLC, Costal Greene well pad. Present at time of inspection was Tyler Dean (Greylock). The site is located in Whiteley Twp., Greene Co. The inspection was related to reported spill of brine to the well pad.

On 6/1/2018, the operator reported a 3-5 gallon spill of brine to the well pad. The operator reported the observation of slightly stain stone on the well pad during the mobilization of hydraulic fracturing equipment onto the well pad. The operator has estimated the suspected brine release to be approximately 3-5 gallons. The operator has visually removed impacted stone from the well pad for disposal and replaced with new stone. Using a soil probe the soil and stone surrounding the location of the reported spill, and all conductivity readings where less than 1 mS. The operator has remediated the brine release inaccordance with § 78a.66(c)(1) for remediation of a release of less than 42 gallons.

2760015 Primary Facility 2018-08-03 Routine/Complete Inspection No Violations Noted The well is properly identified and is equipped for production. The well is producing. The operator is currently drilling on site.
2827700 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted On 01/09/19 at approximately 0835 hours I conducted a routine complete inspection of the site in response to a notification from Greylock that a Modular Above Ground Storage Structure was being constructed. As of the time of inspection the MASS had been constructed.

An area near the MASS showed signs of elevated conductivity compared to background (1.07mS vs 0.2mS). It is recommended the area be assessed for potential produced fluid impacts.

A water line between the MASS and well pad appeared to have been damaged by a vehicle (see photographs). It is recommended the line be pressure tested or repaired prior to use.
