On 01/09/19 at approximately 1000 hours Environmental Group Manager Dave McDermott, Environmental Engineering Manager Kareen Milcic, Water Quality Specialist Supervisor John Carson, Environmental Engineer Mohamed Sam, and I met with Don Supcoe III and permitting supervisor Travis Wendel, both from Greylock. The purpose of the meeting was to discuss the differences between the as built site and Department approved plans. See inspection ID #2831008 for further details.

During the inspection a visible sheen was observed in waters of the Commonwealth in the cellar of the 9MH well. Violations of 78a54 and CSL 401 have occurred.
