No release of a substance causing of threatening pollution of the waters of the commonwealth occurred. All wells on site were shut-in at the time of inspection. Vantage personnel present on site conducting investigation.

2681486 Primary Facility 2018-01-10 Routine/Complete Inspection No Violations Noted On Wednesday, January 10, 2018, I conducted a routine-complete inspection at the Rogersville/Shumaker Well Site located in Center Township, Greene County.

The gas well, 9H (059-26433), and associated infrastructure appeared to be in good working order and the well was labelled.

No violations were identified during the inspection.

2846873 Primary Facility 2019-02-27 Incident- Response to Accident or Event Violation(s) Noted On Wednesday February 27, 2019 I received notification of a release of produced fluid at the Rogersville/Shumaker Well Site located in Center Township, Greene County. The release was identified and reported to the Department the previous day at approximately 1739 hours. On Wednesday morning I conducted an incident inspection to assess the situation. While on-site, I spoke on the telephone with Mr. Corey Giles from EQT Production Company (EQT) regarding details of the release and subsequent remediation.

The release occurred as a result of a dump valve wash-out in the GPU associated with the 9H gas well (059-26433). EQT estimated that approximately 14 gallons of produced water was released due to the wash-out. Approximately four (4) gallons of produced fluid remained inside the GPU, while the remaining 10 gallons had exited the unit and impacted soils behind the unit. EQT's remediation contractor was mobilized and the impacted area was remediate. The impacted area was delineated and the completeness of
