2559149 Primary Facility 2017-01-23 Routine/Complete Inspection No Violations Noted On 01/23/17 at approximately 1400 hours I met with Bryan Morris of Rice Energy to discuss proposed changes to the site. The Ridge Road pad consists of a lower section that currently has wells on it and an upper section that is approximately 3-4’ higher and accessed by a gravel ramp.

Rice Energy requested redline approval to shift the ramp to the southeast and to convert a portion of the upper pad to the elevation of the lower pad to accommodate an oncoming drilling rig. It was proposed that the removed material would be placed next to the existing stockpile and silt sock be added on the eastern portion for erosion control.

The redline changes were approved and it was requested that an as built drawing reflecting the changes be sent to the engineering section in Pittsburgh.

2664752 Primary Facility 2017-11-02 Routine/Partial Inspection No Violations Noted While reviewing an ESCGP 2 major modification/renewal Environmental Engineering Specialist Mohamed Sam observed that the wet pond called out in the approved PCSM plans was not installed. This WQS conducted a routine partial inspection and confirmed the wet pond had not been installed (see photographs).

Violations of 102.5(m)(4) and 102.5(f) have occurred as a result of the Operator failing to install PCSM per the approved plan.
