2824190 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted On Friday January 4, 2019 I conducted a routine-complete inspection at the Lacko Well Site located in Washington Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.

2839597 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted On 2/5/2019 at approximately 1300, I conducted a site inspection of the Lacko Well Site located in Washington Township, Greene County. Department was notified by the operator on Wednesday, January 23, 2019 of a slip on the fill slope just before the entrance to the well pad. During the 2/5/2019 inspection, the following was observed:
  • The act 9 sign located at the site entrance was in good condition.
  • GPUs appeared in good condition.
  • All wells were marked and appeared in good condition.
  • Production fluid/condensate tanks within secondary containment appeared in good condition.
  • Land movement reported by the operator was observed. Additional land cracks were observed along the access road near the site entrance.

The following violations were identified during this inspection and will be assessed against the ESCGP:

102.11(a)1, 102.4(b)1, 78a53, 102.11(a).
