The Operator had the pad just about completed. They had some equipment on site still grading the slopes and the edge of the pad. The BMP's were in place and appeared to be in good condition. The access road is in over-all good condition but does have a few bad spots which should be addressed in the case that emergency equipment needs to drive to the site. The site is in over-all good condition and the Operator plans to begin drilling immediately. Please continue to inform the Department when construction is just about complete so the pre-Op inspections can be completed.

2600365 Primary Facility 2017-06-02 Site Restoration No Violations Noted On Friday, June 2, 2017, I conducted a site restoration inspection at the Patterson Creek Well Site located in Center Township, Greene County. The inspection was conducted in response to an operator submittal of a Notice of Termination (NOT) for the ESCGP (ESX11-059-0055) in accordance with 25 Pa. Code Section 102.22(a).

The Act 9 sign was clearly legible at the site entrance and all required documentation was available on-site. The access road and the well pad are in good condition and the site is stabilized with vegetation. The landowner has assumed responsibility for several areas along the access road and the well pad which are being cultivated per the landowner's intentions.

The secondary containment structure around the production tanks appears to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labelled as required in 58 Pa. C.S. Section 3211(h). 2733600 Primary Facility 2018-05-04 Routine/Complete Inspection No Violations Noted On Friday, May 4, 2018, I conducted a routine-complete inspection at the Patterson Creek Well Site located in Center Township, Greene County.

Gas well 590911 and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.
