2065337 Primary Facility 2012-05-02 Drilling/Alteration No Violations Noted Savanha 635 drilling for Vantage Energy. Were drilling 7 7/8 hole at 12692' ETD 13758'.Casing program and cement returns info. looked good. Site looked good at time of inspection.
2074669 Primary Facility 2012-06-07 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Routine Inspection. The site is currently dormant. The Wells appear to be drilled but not in production. The site is in over-all good condition. A few areas need to be seeded and mulched because the grass did not grow. The drill pits on site still contain water and the site will need to be restored within 9 months of TD. The ESCGP-1 is good for 5 years. The Operator can apply for an extension if that 9 month mark cannot be reached.

2160962 Primary Facility 2013-03-29 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well to Conduct a Routine Inspection. The Marcellus Wells have been drilled but not yet in production. The Department had some concerns about the E/S Controls at the bottom of the fill slope. The BMP's/E/S Controls need to be fixed and put back in good working condition. The collection channel leading to the sediment trap has been compromised by a slide and needs to be re-graded. The Department spoke with the Operator in regards to the site conditions and the E/S Controls.
2230729 Primary Facility 2013-11-25 Routine/Complete Inspection No Violations Noted On 11/25/2013, I performed on inspection of Vantage Energy's Porter Street 1H-10H well pad site. The site is located in Franklin Twp., Greene County. The inspection was performed as a routine inspection during drilling.

2453820 Primary Facility 2016-02-24 Administrative/File Review In Compliance with Policy A release of approximately15 to 20 gallons of produced water brine at the 2H produced water tank unloading point, resulting from a leak in the truck tank that was pulling water from the produced water tank. The incident was reported and remediated the same day. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.
2454250 Primary Facility 2015-12-17 Routine/Complete Inspection No Violations Noted On 12/17/15 at approximately 1230 hours I conducted an incident response inspection resulting from a spill at the Porter Street well site.
2472666 Primary Facility 2016-03-21 Administrative/File Review In Compliance with Policy On Monday, March 21, 2016 a brine spill of approximately five to ten gallons occurred on the Porter Street Well Pad located in Franklin Township, Greene County. A leaking valve within a manifold had released the brine directly into containment. The brine was vacuumed out of containment and disposed off-site. The valve was replaced and all lines and tanks were inspected.

The incident has been determined to be In Compliance with Policy (ICP) and no violations are being cited at this time.

2474912 Primary Facility 2016-03-31 Incident- Response to Accident or Event No Violations Noted On March 31, 2016 a release of approximately five to ten barrels of brine occurred at the Porter Street Well Site. The incident was the result of an operator error and had impacted soil on the pad, along the access road and an area on the outer slope of the access road. Response to the incident was immediate upon discovery. The site remediation is being conducted in accordance with Act 2/Land Recycling Program regulations in Chapter 250 of Tile 25 of the Pennsylvania Code.

The failure to contain the brine release is a violation of Title 25 Pa. Code § 91.34(a). The impact to soils by the brine is a violation of the Clean Streams Law 35 P.S. § 691.401. The operator should assess the current design and consider adjustments to prevent a release from bypassing containment in the future. At the time of my site visit on March 31, 2016, Vantage personnel were actively assessing the current design and considering changes to the set-up, as well as changes in protocol and training for contractors.

2497603 Primary Facility 2016-06-08 Compliance Evaluation No Violations Noted On Wednesday June 8, 2016, I conducted a compliance evaluation inspection at the Porter Street Well Site located in Franklin Township, Greene County. The inspection was performed to review remediation work completed in association with a March 31, 2016 brine release in which an estimated three to five barrels of brine was released and had impacted the ground surface.
2553686 Primary Facility 2016-12-27 Routine/Complete Inspection No Violations Noted

On 12/27/16 at approximately 1130 hours I conducted a routine complete inspection of the Porter Street unit. The Act 9 sign was in place and all required documents were present at the site. The production equipment appeared to be in good condition. The site was well vegetated and I observed no signs of erosion at the time of inspection. There were no violations observed at the time of inspection.
