2055568 Primary Facility 2012-03-19 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Routine Inspection. The Marcellus Wells have been drilled and appear to be in production. The site is in over-all good condition. The E/S controls appear to be in good working condition. The Department discussed a few land owner concerns with this site in the past. There was some minor erosion around the sediment traps. The Operator added some silt fence at the base of the fill slope to insure sediment did not get off site. The Department spoke with the Operator by phone while on site and they plan to begin restoration has soon as the weather permits.
2444598 Primary Facility 2016-01-29 Administrative/File Review No Violations Noted

On 01/29/16 at approximately 0700 hours I conducted an administrative inspection in response to a request for restoration extension submitted by ECA.

Drilling activities for the last well (#2 & #4) were completed on 12/20/2011 according to Department records. The 9 month date for restoration was 09/20/2012. The request for restoration extension was received on 11/18/2015. A violation of Section 3216(c) of Chapter 32 of Act 13 has occurred for failure to restore the site within 9 months of completion of drilling a well. A violation of 102.5(m)(4) has occurred for the permit being allowed to expire.

The extension request is being denied.

2558263 Primary Facility 2017-01-04 Routine/Complete Inspection No Violations Noted

On 01/29/16 an NOV was issued for a violation of 3216(c) for failure to restore the site within 9 months of the completion of the wells. The restoration date was to be 09/20/2012. A violation of 102.5(m)(4) was also issued for ECA allowing the ESCGP to expire.

On 01/04/17 at approximately 1300 hours I conducted a site restoration inspection in response to an NOT submittal by Energy Corporation of America. The Act 9 sign was in place and all required documents were present at the site.

The site was well vegetated and there were no signs of erosion observed. The site meets the requirements for permit termination. The NOT is approved.
