Tanks and separators appeared to be fully functional with no hydrocarbon staining. Secondary containment had excess water (see separate inspection by WQS.

No downhole or surface production equipment appear to be in violation at this time, however separate inspection by DEP WQS is likely to show violations due to surface water control issues

2785562 Primary Facility 2018-09-27 Follow-up Inspection Violation(s) Noted On 06/05/18 Greylock reported a brine release of unknown quantity at the site. On 06/06/18 I, along with WQS Adam Ofstun, conducted an incident response inspection and cited multiple violations. See inspection report # ¿2740986¿. On 06/29/18 WQS Adam Ofstun and Environmental Protection Specialist Kevin Maskol conducted a follow up inspection of the site. According to Ofstun’s inspection report # ¿2747893¿, the brine release appeared to have been remediated. A seep on the southeastern fill slope was observed that was discharging high conductivity water. An ongoing investigation into historic pollution of the site is ongoing under an 11/17/17 COA, which addressed the past usage of flowback pits at the site. EPS Maskol collected sample ¿9539-040¿. Maskol’s sample of the seep suggests the water collected was impacted by O&G activities.

On 09/27/18 at approximately 0825 hours I conducted a follow up inspection. I observed a frac tank on the well pad attached to a hose leading over the southeastern fill slope. Th

2849665 Primary Facility 2019-02-26 Follow-up Inspection Violation(s) & Recurring Violations On 02/26/19 at approximately 0830 hours I conducted a follow up inspection at the site. The Act 9 sign was in place and all required documents were present at the site.

In inspection #2785562 on 09/27/18, a plume of gas and vaporized brine was observed coming from tank #5. On 02/26/19 an additional release was observed. The Department requests Greylock provide an explanation for the continued release from the tank and a schedule for addressing the issue.

A generator set on the well pad was managed within secondary containment. The containment contained a large volume of water. It is recommended the water is removed to maintain 110% capacity.

A slip was observed on the south slope of the well pad. A violation of 102.22(a) has occurred.

The area at the bottom of the southern pad slope exhibited accelerated erosion. The area was not temporarily stabilized, contrary to 102.22(b)(1). Accelerated erosion was observed on the southern side of the well site heading toward UNT to Muddy Creek. Multiple silt socks w
