The Operator is currently drilling multiple Marcellus Wells on this pad. The Department received a complaint in regards to the well site causing accelerated erosion on the down gradient property. The Department did observe E/S concerns at the site. The E/S controls need to be addressed. The water coming off the site has caused the valley to erode at a fast rate. The Department recommends installing a sediment trap and also turn-out ditches to slow down the water as a pollution prevention measure. The Department has spoke with the Operator and the land owner about the issues at the site.

The 3 Marcellus Wells at this site have been drilled and fracked and appear to be in production or will soon be in production. The drill pit that leaked has been restored. All contents from the drill pit have been taken to an off-site disposal facility. The area where the drill pit was located has been re-graded and seeded and mulched. The impacted area appears to be remediated and in compliance. The site is in over-all good condition.

2479727 Primary Facility 2016-04-29 Administrative/File Review In Compliance with Policy Approximately three to four gallons of brine had spilled onto the well pad from a flow line of a GPU. Workers performing maintenance on the unit thought that the line had been drained and inadvertently released the contents of the line when disassembling it. The area was then cleaned and the impacted gravel was removed and placed into sealed drums for off-site disposal. No water was impacted.

It has been determined that the incident is In Compliance with Policy (ICP). No violations are being cited at this time.

2618121 Primary Facility 2017-07-20 Routine/Complete Inspection No Violations Noted On Thursday, July 20, 2017, I conducted a routine-complete inspection at the NV19 Well Site located in Morris Township Greene County.

The access road and the well pad were in decent condition and the site has been stabilized with vegetative growth.

The secondary containment around the production tanks appears to be in good condition and functioning as intended. The gas wells appear to be in good working order and each is labelled as required.

The site ESCGP was terminated in June 2014. Although the operator is not required to keep permit drawings on-site following termination, the operator should consider keeping an as-built drawing with relative PCSM controls on-site.

The operator, however, is required to keep an Emergency Response Plan (ERP) on-site for the duration of operations. It was recommended to the operator in a July 6, 2017 inspection report of NV25 that they should confirm that documentation requirements for all of their sites are up to date and in compliance, otherwise violations would be

2719415 Primary Facility 2018-04-03 Routine/Complete Inspection No Violations Noted On Tuesday, April 3, 2018, I conducted a routine-complete inspection of the NV 19 Well Site located in Morris Township, Greene County.

The NV19BCV gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was labelled.
