2711490 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted On Thursday, March 15, 2018, I conducted a routine-complete inspection at the Pettit No. 5 conventional gas well site located in Center Township, Greene County.

The access road and the pad area were in good condition and the site has been stabilized with vegetative growth.

The earthen berm secondary containment structure around the production tank was observed to be vegetated and in good condition. The production tank appeared to be in good condition. Although the operator is compliant with current regulations regarding production fluid ASTs and secondary containment, the operator should consider utilizing secondary containment constructed with impermeable materials. Although compliant, a release of fluid into the earthen berm containment may results in a violation being issued.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was labelled.

No violations were identified during the inspection.

2850746 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted On Wednesday March 6, 2019 I conducted a routine-complete inspection at the Pettit No. 5 Conventional Vertical Gas Well located in Center Township, Greene County.

The access road and the pad area were in good condition.

Uniform vegetative growth was observed through a thin snow cover on the ground.

The earthen berm secondary containment around the production tank appeared to be in good condition and was observed to be vegetated through the thin snow cover. Although the operator is compliant with current regulations, they should consider utilizing secondary containment constructed of impermeable materials. A release of fluid within the berm can result in a violation being issued to the operator.

The production tank appeared to be in sound condition.

The gas well and associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
