2692825 Primary Facility 2018-02-09 Routine/Complete Inspection No Violations Noted On Friday, February 9, 2018, I conducted a routine-complete inspection at the Perkey No. 3 Well Site located in Center Township, Greene County.

The site is secured with a locked gate. The access road and the pad were in good condition. The area was partially snow covered and significant vegetative growth was observed through the snow cover and in areas without snow cover.

There is no secondary containment structure around the production fluid storage tanks which appeared to be in good condition. There were no visible signs of fluids having been released.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2845230 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted On Tuesday February 5, 2019 I conducted a routine-complete inspection of the Perkey 3 Vertical Gas Well Site located in Center Township, Greene County.

The access road and the pad area were in good condition.

The site has been stabilized with uniform perennial vegetative growth.

The production tank appeared to be in sound condition. There is no,secondary containment utilized with the production tank.

Although the operator is compliant with current regulations, they should consider using secondary containment constructed of impermeable materials. A release of fluid to the ground surface can result in a violation being issued to the operator.

The gas well and associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
