2696991 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted On Wednesday, February 14, 2018, I conducted a routine-complete inspection at the Charles Nelson 2 well site which is located in Center Township, Greene County. The Charles Nelson 2 Well is located on the same pad as the Charles Nelson 3H Oil Well. The Oil &Gas Mapping Site identifies the well as an active vertical gas well which has reported production for the previous years, although information within the mobile SAIR application used to produce this report identifies the well as, operator reported never drilled, which is obviously not the case. There was another well which was permitted for this site, The Charles Nelson 4H, which was proposed and never materialized (PBNM). It is assumed that the well numbers were inadvertently entered incorrectly.

The access road and the well pad were in good condition and the site has been stabilized with vegetation.

There was no secondary containment around the production tank which appeared to be in good condition with no signs of deterioration on the tank or sig
