On 01/04/16 at approximately 0700 hours I conducted a routine complete inspection of the site. The Act 9 sign was in place. All required documents were present at the site.

The site was not restored at the time of inspection. It appeared the pipeline connection had been recently installed to the Pad A. The site was stable.

The Department has reviewed the request for site restoration extension. The restoration date was to be 03/24/16, but a new date of 10/31/16 has been approved for both the Pad A and Pad B.

2494942 Primary Facility 2016-06-08 Routine/Complete Inspection No Violations Noted HORIZONTAL MARCELLUS, Completing final restoration.
2564239 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was vegetated, but additional growth is needed to reach a uniform 70% growth. Small erosion rills were observed on the site (see photographs). The rills had been addressed by Chevron and straw bale BMPs were in place to prevent further erosion. One rill appeared to have previously had straw bale BMPs present, but they had since degraded. It is recommended this location be addressed.
2693630 Primary Facility 2018-02-05 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal by Chevron. The Act 9 sign was in place and all required documents were present at the site. The production equipment on both Pad A and pad B appeared to be in good condition. The site was well vegetated and there were no signs of erosion observed. The site meets and exceeds 70% uniform vegetative cover. The NOT is approved.
