2223538 2013-11-20 No Violations Noted On 11/20/2013, the Department performed an Notice of Termination inspection at the site of Chevron Appalachiaï¾’s Mutnansky Unit 9, 10, 11, 12H well site in North Union Twp., Fayette County. The ESX10-051-0011 is the site permit being inspected for the N.O.T.

There was an Act 9 site ID sign up at the entrance of the access road. The well permits, E&S plan, and PPC plan were located in the box. The access road was in good shape and was gated half way to the site.

The site has been restored. The vegetation covering the reclaimed area looked to be in good shape. Drainage issues in the lower field had been addressed and looked to be working.

The pad area is now fenced off and access to the pad is locked.

The four wells on site were labeled. There are also four tanks in containment and four heater/separator units on site. I did not observe any leaks on this date.

All drilling equipment and supplies not needed for production have been removed from the site.

The N.O.T. has been approved.
