2811655 Primary Facility 2018-11-30 Follow-up Inspection Violation(s) & Outstanding Violations On 7/3/2018, I responded to a spill of approximately 35 barrels of brine water which had released from a defective production fluid tank on the Elias Unit 3 well site. On 11/30/2018 at approximately 1200, I conducted a follow up inspection of the site. I was accompanied by Dave McDermott (DEP). The following was observed:
  • Site access road was in poor condition.
  • Well on site was marked and appeared to be in good condition.
  • Defective production fluid tank on site was placed on a liner and appeared empty.
  • Area with no vegetation was observed where contaminated soil stockpile used to sit.
  • The excavated area had some E&S controls in place, although they did not appear to prevent accelerated erosion and sedimentation on site.
  • Sediment laden water was observed within the excavated area.
  • Sediment laden water was overtopping CFS on site and discharging into an adjacent pond.
