1941202 Primary Facility 2011-01-06 Complaint Inspection No Violations Noted Scott Lux and i completed an on-site inspection and found no evidence of runoff from the site. The E&S Plan was on-site and full implemented. It is possible the run-off came from tile drainage underlying the well pad site, and dumps to the Rt 5 road ditch.
1941890 Primary Facility 2011-01-10 Complaint Inspection No Violations Noted During my on-site inspection today I met with the Lake Erie Energy , dirt contractor Charlie Rahal and his son Ben Rahal to determine how to address the potential field tile, point source pollution from this well pad site. Charlie spoke with the landowner and found out the tile does lay approx. 2.5 ft below the surface of the ground and run under the site. We agreed the best course of action would be to expose, cut and concrete plug the tile both above the site and below the site. The tile will be repaired and flow redirected from the above the site, to go around the site and tie into an exsisting hand dug well nearby. Charlie stated this hand dug well did not have any lines running from it and that it had previuoly served a home that has been torn down for many years. Water entering this tile from above the site will not be impacted by the site. This inspection was conducted in followed to my 1/6/2011 inspection. Charlie will contact PA DEP when the tile is exposed and plugged and before reburying it, so that we can visually confirm the tile has
1941930 Primary Facility 2011-01-06 Complaint Inspection No Violations Noted
1946816 Primary Facility 2011-01-31 Complaint Inspection Violation(s) Noted Today Scott Lux and i completed an inspecton in follow-up to inquires regarding whether or not (2) private water supplies are located withn 1000' of the Dohler # 1 well location. Upon arrival we collected GPS lat and long coordinates for the Mahoney and Counassr private water well at 191 Hidden Lane and 278 Arden Lane respectively. We also collected GPS lat and long coorindates for the Dohler # 1 well marker stake. This GPS data was downlaoded to TerrianNAV and we determined the Mahoney water well was found to be 1100' from the Dohler # 1 well marker stake. However the Counassr water well was 750' from the Dohler # 1 well stake marker, requiring the permit applicant to list this residence on the Permit Application, and provide written notification of the pre-drill water sample opportunites to the occupants @ Counassr. The permit applcation failed to meet these requirerments, constituting a violation of Section 201 (b) of Act 223. While on the Dohler # 1 Well site we noted that recent earth moving had taken place on the access road.
1947833 Primary Facility 2011-01-31 Routine/Complete Inspection No Violations Noted Today Scott Lux and I completed an onsite inspection of this well pad site to collect exact lat and long coordinates for the well stake for use in determining if nearby private water wells are within 1000' of the well stake. Upon inspection we found the west end of the access road had been scraped to remove the top soil, to provide a base for rock. The top soil was piled in a manner to reduce run-off and slit fence was in place . There was 12" on snow and ice on the ground . The E&S Plan and slit fence was in palce for the site. No violations where found.
1947850 Primary Facility 2011-02-04 Complaint Inspection No Violations Noted Today Scott Lux and I inspected this well site to visually confirm the location of field drain tile that runs under the well pad site and check any changes to the project. Upon arrving we met with LEEP Rep's including Adam Schwindt and Charlie Rahal and discussed the scraping of the top soil off the west end of the access road and looked at the field tile that Charlie had dug up to conifrm it's location. As we looked at the tile Charlie explained the fist step was to trench and lay pipe to connect the drain tile to when he cuts it off; this new pipe will divert the drain tile around the well pad site and dump it to a wet area nearby. Once Charlie has the the new pipe laid he will call me to setup a visual inspection f the cutting and plugging of the tile. The discharge water from the tile will flow onto large rip-rap to prevent E&S problems. The top soil scraped from the access road bed was piled in a manner to prevent E&S problems and E& S measures where in place as required. no violations.
1948913 Primary Facility 2011-02-10 Complaint Inspection No Violations Noted Today I completed a follow-up inspection to a complaint received 1/3/2011 that alleged heavy slit laden runoff was entering the road ditch of Rt 5 from this gas well site. Investigations on previous dates found the possibilty of a field drain tile that ran under the well site may have been the source of the runoff. It is important to note that there was no evidence that this was the source, however it was determined to be the only possible source, if indeed the problem of slit laden runoff exsisted. Previoulsy DEP had required the operator locate the tile above and below the well pad site and cut it. After being cut we required the operator contact us to verify it was cut and plugged with concrete before burying. During my inspection today I witnessed the dirt contractor Charlie Rahal, plug and cement the field drain tile that runs under this site. Charlie then buired tile. I took photo's of this plugging project . Charlie will also install slit fence around the earth disturbance created by the burying of the new field tile.
1950129 Primary Facility 2011-02-14 Complaint Inspection No Violations Noted Today's inspection was in follow-up to the complaint received on 1/3/11 regarding E&S problems allegedly orginating at this well site, that is under construction. At DEP's request, the operator recently installed a re-routed field drain tile to carry water around the well pad site. On 2/10/2011 I had meet the operator on site and explained they must install slit fence around this earth disturbance. Upon inspection today I found this work completed as required. No violations.
1950141 Primary Facility 2011-02-14 Complaint Inspection No Violations Noted During my inspection today I verified the operator had installed the slit fence around the earth distrubance surrounding the recently buried field drain tile that diverts water around the well site. I had required thsi work as part of my 2/10/11 inspection. No violations.

1951513 Primary Facility 2011-02-22 Routine/Complete Inspection No Violations Noted Today Scott Lux and I met LEEP representatives Charlie Rahal and Adam Schwindt on the Dohler # 1 well site to review what action has taken plac to address E&S problems identified during Scott Lux's 2/18/11 inspection of the site. See my 2/22/11 inspection report for details.
1951867 Primary Facility 2011-02-24 Follow-up Inspection No Violations Noted Today I meet with Charlie Rahal to verify the following corrective measures that DEP required duirng our recent inspections: 1. The diversion pipe around the west edge of the site was installed and properly tied-in to the drain tile that runs to Rt 5. I also verifyed that the portion of the diverison ppipe that runs under the access road was inside a section of steel pipe to prevent crushing by heavy equipment. 2.The field drain tile that is located in the sump at the north edge of the site was cut-off and plugged with cement. 3. The field drain tile located in the hand dug hole located just off the site to the north was cutoff and plugged with cement.
1952816 Primary Facility 2011-02-28 Compliance Evaluation Recurring Violations The purpose of my inspection today was to determine if the installed E&S controls prevented pollution to water of the Commonwealth during this time of heavy rain and snow melt. Upon inspection I found that due to an extraordinary amount of rain and snow melt, sediment laden water did enter the waters of the commonwealth. A Notice of Violation will be issued. See my 2/28/11 inspection report for the details.
1954369 Primary Facility 2011-03-03 Compliance Evaluation No Violations Noted Today Jon Scott and I completed a compliance inspection of the Dohler # 1 site to determine if the violation of Section 401of the Clean Stream Law identified on 2/28/11 still exists. Upon arriving we found the runoff from heavy rain and snow melt had stopped, the E&S controls in good repair and all water retained on the site. The condition that allowed the runoff to flow across this site and overflow the E&S controls still exists. Our observation of the site and surrounding area indicated that in the event of another heavy rain the site could again be flooded and the E&S controls overloaded causing sediment laden water to flow to the Rt 5 road ditch. Immediately following the inspection I contacted both Adam Schwindt and Charlie Rahal by phone today and explained this concern. See my 3/3/11 inspection report for more details.
1958692 Primary Facility 2010-03-17 Follow-up Inspection No Violations Noted Today Scott Lux and completed a follow-up inspection, to ascertain what steps have been taken to address the problem of heavy run-off flowing from the vineyard, south of the well pad site that was identifyed during my 3/3/11 inspection. Upon inspection we found the opeartor had installed a long trench on the south end of the site, between the top soil pile and the vineyard, running west to east, to capture and direct the vineyard runoff above the well pad site, and discharge to a point off-site the well pad site. This water flows from the trench into a section of slit fence and hay bails, and down through a large grassy area. This E&S control appears to have greatly reduced the run-off flowing onto the well pad site. No violations.
2007045 Primary Facility 2011-09-28 Site Restoration Pending Today's inspection was in follow-up to my 9/21/2011 inspection to confirm if the barren ground surface I discovered on 9/21, overlays the buried waste pit. Upon arrival I dug a hole in the barren area to a depth of 10" and found the pit liner. I found no water or liquid laying on top of the liner on in the holes I dug. On 9/28 and 10/3 , I contacted the company owner/Operator John Sturgill and dirt contractor Charlie Rahal and told them what I found and explained that after conferring with my supevisor Scott Lux, we determined the site reclaim was not finished. We required the top soil that was barren be removed to a depth of the top of the liner and new soil be carried in, seeded and mulched. I explained they could complete this work either now or next Spring when the ground dries up. I further explained that if the cover did not grow back by mid-summer 2012 - additonal steps would need to be taken. The status of the reclam approval is Pending.
