2707970 Primary Facility 2018-03-20 Incident- Response to Accident or Event Violation(s) Noted On 3/19/18 at 1345 hrs., I received the following email from Tim Oknefski with ARG Resources:

"Today around 11:00 AM ARG Resources, Inc. had a water leak right next to the location at well #261 WT. 3783. ARG has since shut this line off and called a backhoe, vac truck and welder to the site to fix the problem. The source of the leak was a hole on the 3 inch steel produced water line. This line has produced water only in it. Approximately 3 barrels of water was lost and ARG recovered approximately 1 ½ barrels of this water. In order to fix this problem ARG had to weld the hole closed on the steel pipe. That has been completed. Now we will order some hoppers to the site so that we can excavate this soil and dispose of it properly. Some water did go about 50 feet north of the site but was contained at that point. Water did not make it any further than that. The line is being opened now because the problem has been fixed."

2814258 Primary Facility 2018-12-04 Follow-up Inspection Outstanding Violations - Viols Req'd On 12/4/18 at 0852 hrs., I conducted a follow up inspection of ARG Resources, Inc.'s WT 3783 261 well site (API 047-23898) in Highland Twp., Elk Co. Of particular concern were the violations cited in inspection ID 2707970 relating to a release that occurred at the site. The impacted soil has been excavated. Sample location markers are present along the spill path. Violation ID numbers 811489, 811490, and 811491 from inspection ID 2707970 remain outstanding and are being cited again for this inspection.
