| 2752087 | Primary Facility | 2018-07-16 | Incident- Response to Accident or Event | Violation(s) Noted | On 7/13/18 at approximately 1345 hrs., I received a phone call and email from Tim Oknefski with ARG Resources, reporting a crude oil and injection fluid release at their Lamont 011 well (API 047-21349) in Highland, Twp., Elk Co. According to Mr. Oknefski, approximately 5 gallons of oil and 20 gallons of production fluid were released from a 2 inch line. The release was mitigated and contained. A vac truck was on site as well to remove the fluids. Mr. Oknefski stated that the impacted soil was still in place and would be removed upon receipt of a roll-off dumpster.
On 7/16/18 at 0905 I conducted an inspection of the release site. The area had been backfilled with the impacted material. Soil conductivity ranged from 2.0 to 3.0 mS. The area had not yet been seeded and mulched. The release constitutes violations of 78.54, CSL 402(b), SWMA 301, SWMA 302(A), and SWMA 610(1). On 11/20/18 at 0905 hrs., I conducted a compliance evaluation of the release site. Upon inspection, impacted material had been removed from the site. Soil conductivity readings around the release site were all between 100-400 uS/cm, equivalent to background readings. Site stabilization could not be evaluated at the time of this inspection due to snow cover. This inspection resolves violation numbers 822306, 822307, 822308, 822309, and 822310 from inspection ID 2752087. There are no additional violations noted for this inspection. |
