Producing combination oil & gas well with electric pump jack. Leaking stuffing box at wellhead. 3'x12' of paraffin/oil stained ground at wellhead (Pic #1). No well tag at wellhead. Separator (2 on site) plumbed to a 70 bbl open-top steel tank (3/4 full) in an earthen dike. 70 bbl tank plumbed to 100 bbl steel tank with green flex hose on drain valve; also in dike. Dead vegetation inside of dike (Pic #2) and in roadside ditch (Pic #3). No oil staining on vegetation. A spill or leak of brine of unknown volume has occurred at the tank battery, dike and/or roadside ditch.

Violations noted for: 1) discharge of residual waste to ground (SWMA301) at wellhead and tank battery/roadside ditch 2) no well tag (OGA3211(a)) at wellhead

WQS spoke with operator same day, who will be removing contaminated soil at wellhead, inside dike and roadside ditch. Operator will also install well tag at wellhead to correct violations.
