2743639 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at XTO Energy Inc PA TRACT A on June 20, 2018 with production supervisor Paul Hahn of XTO. XTO recently submitted SPUD notices to the Department for the following wells: 035-21316, 035-21317, 035-21318, 035-21319, and 035-21320.

According to Mr. Hahn, XTO is currently drilling and setting a 24" conductor, 42' in the ground for each above listed well. Once completed, a crew will be moving in to install containment and a top-hole rig will be moving onsite for drilling. The top-hole rig will first start drilling and setting casing for well 035-21320 until reaching the kickoff point (KP). Once completed, the rig will move to the next well and proceed until all 5 wells are KP. Then another rig will move onsite to perform the horizontal drilling for each well.

There are currently (2) producing wells located onsite (035-21217 and 035-21218). There are (2) gas production units (GPU) and (2) 400 bbl. produced fluids tanks located in the sa

2750601 Primary Facility 2018-07-09 Drilling/Alteration No Violations Noted The weather is 82 Deg. F and sunny at the time of inspection. Follow up inspection of XTO Energy Inc., PA Tract A Pad, well # 5H on July 9, 2018.

Drilling of the 17-1/2¿ hole (surface 13-3/8¿ section) was in progress at the time of inspection. Current depth is 893 ft. While drilling top hole on air, ground water, oil or gas have not been detected. Cement job log data (post treatment report) was included to this inspection report, that was sent to DEP via e-mail on July 10th. No violations are noted at time of inspection. Wellsite looks neat and well organized. Discussed a follow up inspection during or after cement job of the intermediate string section of the well, that is scheduled on July 12th. 2752145 Primary Facility 2018-07-12 Drilling/Alteration No Violations Noted The weather is 85 Deg. F and sunny at the time of inspection. Follow up inspection of XTO Energy Inc., PA Tract A Pad, well # 5H on July 12, 2018.

Drilling of the 12-1/4¿ hole (intermediate 9-5/8" section) is on hold at the time of inspection. Current depth is 1936 ft. While drilling top hole on air, ground water, oil or gas have not been detected. Got twisted off while drilling at 7 am on July 12. Currently fishing tools are in the hole. No violations are noted at time of inspection. Wellsite looks neat and well organized.

2773748 Primary Facility 2018-08-31 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at XTO Energy Inc Pa Tract A well Pad on August 30, 2018.

The site sign, well permit, ESCGP maps, PPC plan, and site specific emergency response plan were onsite for review. The PPC plan was last revised in March 2017. The Department recommends updating the PPC plan if needed with the recently new wells drilled, but not yet completed onsite. The ESCGP (ESX11-035-0010) for the construction of this well pad and access road was terminated by the Department in September 2016.

Two Marcellus wells are currently producing on this well pad (035-21217 & 035-21218). The following five wells recently had the tophole drilled (035-21316, 035-21317, 035-21318, 035-21319, 035-21320). Each well is covered with a metal plate. The tophole rig has moved off location. A new rig will be moving onsite in the next few weeks to drill the horizontals.

Two GPUs and (1) 400 bbl produced fluids tank is located in the same secondary containment. Approximat

2792635 Primary Facility 2018-10-18 Drilling/Alteration No Violations Noted On October 19, 2018, Department representative Brad Kline and Sue Herzing conducted a routine site inspection at XTO Energy Inc. PA Tract 8546H. XTO company man gave us a tour around the well pad. The operator at the time of inspection was tripping out of the hole. Rig and chemicals being stored on site were in a bermed secondary containment. Rig #583 was on well number 3 of 5. Drill cuttings are being collected and combined with sawdust. The cuttings are then being screened. 2 previous wells drilled are currently shut in(035-21217 and 035-21218).

No violations noted.

2807794 Primary Facility 2018-11-27 Drilling/Alteration No Violations Noted On November 27, 2018, Department representative Brad Kline conducted a routine site inspection at XTO Energy Inc. PA Tract 8546H. XTO safety and environmental person Paul gave me a tour around the well pad. The operator at the time of inspection was tripping out of the hole, as they have reached total depth. Rig and chemicals being stored on site were in a bermed secondary containment. Rig #583 was on the 9H well. Drill cuttings are being collected and combined with sawdust. 2 previous wells drilled are currently shut in(035-21217 and 035-21218).

E&S controls and condition of the pad could not be evaluated due to the snow cover on the ground.

No violations noted.

2812701 Primary Facility 2018-12-07 Routine/Complete Inspection No Violations Noted On December 7, 2018, Department representative Brad Kline and Jared Jacobini conducted a routine site inspection at XTO Energy Inc. PA Tract 8546H. XTO safety and environmental person Paul gave us a tour around the well pad. The operator at the time of inspection performing a slip cut. Rig and chemicals being stored on site were in a bermed secondary containment. 2 previous wells drilled are currently shut in(035-21217 and 035-21218).

No violations noted.

2833874 Primary Facility 2019-01-30 Routine/Complete Inspection No Violations Noted I arrived on-site and met safety rep Paul Morvant and company man Jerry Niedert. Patterson rig 583 was active on the site, drilling at 15458' on the 85H well. Paul accompanied me on my inspection. Due to 4-6" of snow, assessing the sitewide containment was difficult. Foam berms appeared to be in good condition, although they were slightly flattened in traffic areas. Chemical containment appeared clean and well managed, as did the cutttings backyard.
2839037 Primary Facility 2019-02-07 Drilling/Alteration No Violations Noted On February 7, 2019, Department representative Brad Kline conducted a follow up inspection at XTO Energy Inc. PA Tract 8546H. The follow up inspections is due to the condition noted in Inspection No. 2836543. A tour around the well site was given by Safety Manager Paul Morvant.

The spill containment around the drilling rig was about half rebuilt at time of inspection. There was a vacuum truck on-site sucking water from containment and around the well pad. The puddles that had a film or sheen on them in the earlier noted Inspection Report were vacuumed up or planned to be. The dry chemicals have been placed in a separate spill containment. The containment has holes in it but is being addressed. The chemical totes have been staged in another area of spill containment in a more organized manner. Field conductivity measurements were taken in various areas of the well pad. The highest reading near the drill mud return area yielded a 560 uS/cm reading.

2849601 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted On March 6, 2019, Department representative Brad Kline conducted a routine site inspection at XTO Energy LLC PA Tract 8546H Well. No E&S issues were observed during the time of inspection. Vegetation could not be evaluated due to snow cover. At the time of the inspection, christmas trees were being set up on the wells. There were 3 frac tanks on the well pad. Containment was being set up for the tanks. There did not appear to be any drilling mud spilled on the pad from the top down rig that was on the pad previously.

No violations noted.

2856561 Primary Facility 2019-03-20 Routine/Complete Inspection No Violations Noted On March 20, 2019, Department representative Brad Kline conducted a routine site inspection at XTO Energy Inc. PA Tract 8546H Well. Jim Matheny of Matheny Energy gave a tour around the site. No E&S issues were observed during the time of inspection. The site appeared to be stable. They were working on preparing to frac the 7H and the 9H wells. The rig was shutdown at the time of inspection, due to an issue with the 9H well. There were 8 production fluid frac tanks and 4 freshwater tanks. The containment around the frac tanks and chemicals were in good condition.
