There are 2 producing wells on this pad. Well # 1031H is 20" conductor that has been drilled to 40 ft. depth, not cemented and then plugged in 2012. 0% combustible gas was detected in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2824549 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted On January 9, 2019, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 285 Pad F. ESX100350008(1) permit covers Pads F and G but only Pad F was inspected.

No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 2 producing wells and 2 plugged wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. The 2 plugged wells have monuments. There are 2 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment. The secondary containment for the production fluid tanks had holes in the polyurethane (Photo 5). The Department recommends repairing or replacing the liner.
