The site sign, 911 emergency contact sign, and E&S weekly inspection reports were onsite. There are 5 plugged wells and 1 inactive drilled well onsite. Well (035-21205) was granted regulatory inactive status on 8-21-13 and expires 8-21-18. All 5 plugged wells have a monument and are labeled. I did not observe a labeled well tag on the inactive well; I recommend labeling the inactive well. The site is stable. Temporary E&S controls have been removed. The pad remains at full built-out.

According to the PCSM Appendices last updated in December 2016, this location should have the well site restoration reports and extension request submitted by the end of 2017 calendar year. To date, these reports and/or requests have not been submitted.

The six wells located on this pad have expired well permits. According to Chapter 78a.65 the operator shall restore the land surface areas within 9 months of drilling completion. I recommending rest

2814410 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted On December 12, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating LLC. COP Tract 342 Pad A. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover. The pad has not been restored.

There are a total of 5 plugged wells located on this well pad with monuments. There is 1 well in regulatory inactive status. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed.

2838296 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted The weather is 45 Deg. F and cloudy at the time of inspection. Routine inspection of Alta Resources, LLC., COP Tract 342 Pad, Well A-1002H, on February 7, 2019. There are 1 inactive and shut in well, and 5 plugged wells on this pad.

This well is 120 ft. deep plugged conductor, date plugged 11/23/2012. 0% combustible gas was detected in the area immediately surrounding the well. No violations are noted at time of inspection.
