The site sign, well permits, emergency response plan, monthly tank maintenance log, and weekly E&S inspection logs were onsite and available to review. Please note that the incorrect ESCGP number is being reported on the weekly E&S inspection log. Last E&S inspection was conducted on 3/19/18.

There are 3 drilled Marcellus wells located on this well pad (035-21178, 035-21179, 035-21180). Well 035-21179 is currently in regulatory inactive status. Each well is properly tagged. All well cellars are full of water and were frozen. There is one GPU and one line heater onsite in containment. Three produced fluids tanks stations were stationed in secondary containment. Water within containment was frozen. Several corrosion spots were observed on the exterior of the produced fluids tanks. Well # 1014H has Regulatory Inactive Status that expires on 06/17/2019. Production by intermediate (4-1/2¿ X 9-5/8¿) annulus vent is shut in. 0% combustible gas was detected in the area immediately surrounding the well. No violations are noted at time of inspection. This well is in Regulatory Inactive Status at the time of inspection. Upon arrival to the wellsite, production by intermediate annulus vent (4-1/2¿ X 9-5/8¿) was open to the atmosphere. After inspection production operator closed this vent. Intermediate by surface annulus vent (9-5/8" X 13-3/8") was open and vented to the atmosphere. Verified following pressures during inspection: annulus vent (4-1/2¿ X 9-5/8¿) is 0 PSI, annulus vent (9-5/8" X 13-3/8") is 0 PSI, production tubing is 0 PSI, backside production casing pressure is 0 PSI. 0% combustible gas was detected at the annulus vent; 0% combustible gas was detected in the area immediately surrounding the well.
