The site sign, well permit, and emergency response plan were onsite. There is one vertical Marcellus well drilled on this pad (035-21108). The well head is properly tagged. Frozen water was observed in the well cellar. Two produced fluids tanks are stationed in an earthen dike. No water was present within the dike. Woody vegetation was present within the earthen dike. The Department recommends removing the woody vegetation in efforts to prevent any future impacts this vegetation may cause to this secondary containment. Several corrosion spots were observed on the produced fluids tanks. The Department recommends the operator continue to monitor the tanks and replace them if necessary. One GPU is stationed onsite in an earthen dike. No water and no vegetation were observed in this dike.

2754331 Primary Facility 2018-07-20 Incident- Response to Accident or Event Violation(s) Noted Department representative Sue Herzing conducted an incident related inspection on July 20, 2018 at Alta's COP Tract 653 Pad 1000. Onsite during the inspection were the following: Rick Hall of Alta, Dan Long of DCNR, Ryan Bidelspach of GES, and Tobias staff for excavation purposes.

Mr. Hall reported a brine spill to the Department on July 19, 2019 at approximately 1445. According to Mr. Hall, an environmental consultant was onsite collecting 26R samples when they found a spill near the produced fluids tank. Alta was immediately contacted and then the Department was immediately notified of the spill.

The spill occurred from a faulty closed valve on the spill pot which is connected to the load out pipe from the produced fluids tank. According to Mr. Hall both valves on the load out pipe and spill pot were closed but brine water was slowly dripping out of the spill pot valve. The spill pot itself was full of brine water and eventually overflowed causing the release of brine fluids. Two (200) bbl produced flu
